How to Join

 Are you interested in joining the Association? 

Submit the following requirements to the AFAD office:

1. Accomplished AFAD Membership Application Form [Download]
2. Accomplished Personal Information Sheet/s (owner and authorized representative) [Download]
3. Letter of Intent addressed to the PRESIDENT through CHAIR, MEMBERSHIP COMMITTEE
4. Letter of Endorsement from an AFAD Member in good standing.
5. Certified True Copies of the following: (Certified from the source)
   1. License to Operate
   2. License to Import (if applicable)
   3. Business/Mayor’s Permit
   4. DTI (DTI Registration) or SEC (SEC Certificate, By Laws, Articles of Incorporation) PAPERS
6. NBI Clearance of company president/owner (Original)
7. Photos of Gun Store Premises
