LTOAPF & PTCFOR requirements

LTOAPF Requirements

  1. Accomplished LTOAPF Application Forms (2 copies) – Download forms here or here: back up link
  2. Regional Trial Court (RTC) & Municipal/Metropolitan Trial Court (MTC) Clearance* OR National Police Clearance or NBI Clearance
  3. Neuro-Psychiatric Clearance from the PNP Health Service at Camp Crame or PNP Regional Offices OR any of the following: NP Clearance from DOH Accredited Government Hospitals or AFP & PNP, PNP HS validated NP result
  4. Drug Test Clearance from the PNP Crime Laboratory at Camp Crame or PNP Regional Offices OR from accredited drug testing centers in areas where there are no PNP CL.
  5. Gun Safety and Responsible Gun Ownership Seminar Certificate from a PNP FEO Accredited Gun Club OR Certification from PNP HRDD for active PNP personnel or Authenticated copy of Retirement Order for PNP and AFP retirees or Certification from Head of Unit/Office of the AFP and other Law Enforcement Agencies indicating that the said applicant who is an organic member of their respective Units/Offices had undergone Gun Safety Training/Seminar.
  6. National Police Clearance or PNP Directorate for Intelligence Clearance OR City/Municipal Police Clearance.
  7. NSO Birth Certificate or Passport OR DFA Certificate of Filipino Citizenship or Oath of Allegienace/Certificate of Registration from BID for Dual Citizens or Birth Certificate from Local Civil Registry
  8. Proof of Latest Billing Address – address similar to address on LTOAPF Application OR any of the following: Proof of billing in the name os the spouse coupled with a Marriage Contract or Barangay Clearance for LTOPF purpose
  9. One (1) Valid Government-Issued ID
  10. Seven (7) pieces 2×2 ID pictures
  11. Proof of Income: Pay Slip or Income Tax Return (ITR) or W2 for government employees/2316 Form for civilians or Certificate of Pension or Bank Certificate of Deposit or Land Title/Certification from Brgy. Chairman & Chief of Police for proof of livelihood (for farmers only) or Other documents that may prove source of income.

Picture Taking at the FEO, Camp Crame or in PNP Regional Offices (once requirements are complete)
* Requirements for MTC & RTC: Prosecutor’s Clearance, Brgy. Clearance, Police Clearance, Residence Cert., 2×2 Pictures
* Requirements for DI Clearance: Residence Certificate

Other Accepted Requirements for Specific Occupations:

For Private Employees/OFWs, add:

Certificate of Employment, Company ID; Overseas Employment Certificate (OEC) from POEA (for OFWs)

For Businessmen/women, add:

Business Permit or SEC or DTI or CDA registration

For Farmers, add:

Land CTC 

For retired employees, add:

Retirement Order or appropriate documents 

For Government Employees, add

Appointment Order or Oath of Office, Government Unit ID 

For PNP/AFP/BJMP/BFP, add:

Appointment Order, Certificate of Duty Status, Non-Pending Case Clearance, Unit ID 

For Military Reservist, add:

J9 Clearance 

 

 PTCFOR Requirements

  1. PTCFOR application form (Issued on site)
  2. Copy of authenticated License to Own and Possess Firearm (LTOPF)
  3. Copy of authenticated firearm Registration
  4. NBI Clearance
  5. Threat Assessment (for new applicants) or previous PTCFOR for renewals

 

Additional Requirements for Specific Occupations:

For Businessmen/women, add:

Business Permit/SEC/DTI/ITR

For Government Employees, add:

Government Office ID, Appointment Order/Oath of Office, Duty Status, & Non-Pending Case 

For Senior Citizens, add:

Senior Citizen ID 

For Reservists, add:

Duty Status, Non-Pending Case, AADT, J9 Clearance & Reservist ID
